Your first visit to DentalWest establishes the vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, such as your medical history, and allow you time to aquaint yourself with Dr. Leavens. You may find answers to many of your questions by browsing through our website. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There is also biographical information about Dr. Leavens. You can even save some time at your first visit by downloading your patient forms from the links above. If you wish to fill these out in the office, please arrive 15-20 minutes early for your appointment. We look forward to meeting you!

Insurance and Billing

We accept most traditional insurance plans, but please contact our office prior to your appointment to verify acceptance of your plan. DentalWest does not participate in Health Management Organizations, however, we will be happy to file your insurance claims for you. We accept checks, cash, and credit cards. We also offer interest-free and flexible payment plans through CareCredit. Please ask us for details.
